Please complete this form to submit your reservation. Once received\, the online course registration details will be sent to you from our certified instructor. Here are the steps to complete: \nPart 1: Online Course (Due by March 1st)\nCOMPLETE this form to receive online course access. Specific registration and certification fee details will be emailed directly to those members who respond.\n*Part 1 is REQUIRED in order to participate in Part 2 and receive certification* \nPart 2: In-person Skills Certification (March 14th)\nUpon completion of the online course in Part 1\, you will be eligible to participate in our in-person skills test. Part 2 test details:\nSkills Event Date: Saturday\, March 14 8:45 am until completed\nLocation: Meadow Park Building at 2333 Meadow Street\, SLO\nCertifications: Adult and Child First-Aid and CPR \nSuccessful completion of both portions will result in your certification which will be valid for two years. \n**NEW: You can participate in the skills only session if your certification is still valid and you are wanting a refresher.
